I have used Photoshop CS3.
I would say what I did,but really I just doodle with things until I felt like i liked what the effect was.I had used a lot of burning & dodging tools,blurring effects,lighting,hue/sat settings.
In the reference Image url is the before photo of my friend,and this is the after math =)
